HMEC providers: It may be helpful for you to review the BREATHE preparedness resources for Individuals first before reviewing the resources for HMECs since these documents are referenced in the HMEC resources.
Please use this checklist as your guide when sharing BREATHE resources with patients. By doing so, you will ensure that all essential BREATHE resources and items were relayed to your patients. You can file the checklist along with other records in patients’ case management files.
These are suggested responsibilities, preparedness and response actions for HMECs made through work done by the Respiratory Emergency Planning Committee (Franklin & Williamson counties, Illinois). Please work with your community emergency planners (local health department, emergency management, or possibly area hospitals) to discuss and tailor this to your local jurisdiction.
This is a suggested decision-making, communication pathways flowchart for HMECs and their patients during a power outage or other type of emergency. Ideally, it would be helpful to review this with your local emergency planners. Note: this is a legal-sized document – 8.5 x 14.
